NDIS & community support · Melbourne

Support that shows up, and stays.

When someone you love has been let down before, you need more than a service. You need people who turn up consistently, build a real relationship, and stay alongside you through the hard parts as well as the good. That is what we are here for.

Total Care Foundations: people of all abilities coming together around a home and a heart. Care that feels like home.

Why we exist

For the people the system keeps missing.

Total Care Foundations was not built from a business idea. It grew out of more than twenty years spent alongside people living with disability, mental health challenges, housing instability and crisis, and out of lived experience of navigating those systems from the inside.

People deserve more than a provider who delivers a service. They deserve someone who stays.

So we built an organisation around that. One where people feel safe, respected and genuinely cared for, and where support does not stop the moment things get hard.

What we do

Support built on trust, not tasks.

We specialise in supporting people with complex needs who need consistency and a real relationship before progress can happen. Here is how that takes shape.

Our hero service

Relationship-led complex support

For people who need consistency, trust and genuine connection before anything else can move. The support is not defined by the tasks. It is defined by the relationship.

Everyday support

Community access & daily living

The practical, everyday support that keeps life moving. Getting out into the community, appointments, routines, and building capacity at a pace that suits the person.

When families need a breath

Respite & short-term support

Short-term and emergency support for people at risk, and a break for families carrying a lot. Delivered with the same care as everything else we do.

Someone in your corner

Advocacy & support coordination

Help to navigate a system that is hard to navigate alone. We speak up when it matters, and we stay involved when things get complicated rather than stepping back.

Why people trust us

Built through relationships, not marketing.

We stay

We remain when others step back.

Many of the people referred to us have experienced provider breakdown or support that fell away when things got difficult. Rather than withdrawing when complexity increases, we stay involved. People have been with us for years, and many ask for us by name.

Trusted by professionals

Referrals from people who put their name on it.

For more than seven years we have grown through GPs, psychiatrists, behaviour support practitioners, hospitals, support coordinators and community networks. A referral partner puts their reputation on the line every time they recommend a provider. They keep recommending us.

Our strongest advantage is not the breadth of our services or a marketing budget. It is the confidence families and professionals have that we will show up and stay. More about how we work →

One family, growing together

The Total Care Family.

Support does not fit neatly into funding lines. People need help across different parts of life, so we have grown into the services that support the whole person and their home, all under one roof you can trust.

  • NDIS & community support
  • Total Care Community Aged Care
  • Total Care Home Assist (gardening & property maintenance)
  • Spotless Solutions (cleaning & mobile laundry)

For coordinators & health professionals

Support you can put your name to.

If you have someone who has been through provider breakdown, or who needs a service that will genuinely stay the course, that is exactly who we are for. We communicate openly and we keep you in the loop.

“Someone I trust to genuinely show up, stay involved, and help rebuild stability.”

The kind of provider a family, GP, hospital or support coordinator reaches for when a person has been failed before.

Tell us who you have in mind.

Whether you are a family, a participant, or a coordinator, tell us a little about the person and what they need right now. We will take it from there.
